I hereby authorize and direct Mellina Animal Hospital to hospitalize/board my pet(s). *

Your pet will need to be up to date on core vaccinations according to Texas State Law. If not, these vaccinations will be administered following a complete physical exam upon admission to the hospital at my expense.

If my pet is receiving medication, the staff at Mellina Animal Hospital will administer the medication while in the hospital. There may be an additional charge for this service depending on the frequency of medication and type of medication. If I forget to bring the medication with me, Mellina Animal Hospital will refill enough of the prescription to cover my pet's hospital stay, and I will be charged the normal pharmacy fees.

My pet will be checked for fleas while here. If evidence of flea infestation is found, my pet will be de-flead by Mellina Animal Hospital staff, and I will be charged for this service.

If my pet requires medical attention while hospitalized, Mellina Animal Hospital will perform diagnostic and/or treatment procedures as deemed advisable or necessary for my pet, and I will be charged for these services. We will do everything we can to contact owners before treatment is administered. However, in the event owners are not available, we will do minimum treatment until owners are reached.

Payment is to be made by the time I take my pet home. I agree to pay in full for services rendered, including those deemed necessary for medical complications or unforeseen circumstances.

Hospital and boarding fees are based on the times that Mellina Animal Hospital staff are involved with the care and cleaning of your pet. Office hours are:
Monday - Friday: 7:00 AM - 5:30 PM (closed on Tuesdays from 12:00pm - 1:30pm for lunch)
Saturday: 8:00 AM - 12:00 PM (Noon)
Sunday: Closed
